AB OBJECTIVE: The study aims to determine the susceptibility patterns of
seven (7) oft-prescribed antimicrobial agents for the treatment of
gonorrhoea in Kumasi, Ghana. METHODS: One thousand, one hundred and
eighty-seven (1187) patients from whom specimens (high vaginal, cervical
and urethral swabs) containing Gram negative intracellular diplococci
(GNID) amidst polyphonuclear leukocytes were identified constituted the
study population whilst the specimen, the study material. Identity of N.
gonorrhoeae was confirmed by colonial morphology, positive oxidase test
and sugar fermentation pattern. Penicillinase producing N. gonorrhoeae
(PPNG) strains were determined by the Intralactam Strip method. Minimal
Inhibitory Concentrations (MICs) of seven (7) antimicrobial agents were
determined by the agar plate dilution method. RESULTS: Of 1187 N.
gonorrhoeae strains isolated, 93.93% were PPNG. The disc diffusion
Antibiotic sensitivity test results showed that Ceftriaxone (100%) and
Norfloxacin (99.50%) were the most effective antimicrobials against
gonorrhoea, inhibiting all 300 selected PPNG strains at MICs of < or =
0.03 mg/L. and < or = 0.5 mg/L., respectively. Over 60% of the isolates
were sensitive to Erythromycin, Spectinomycin, and Tetracycline.
Isolated PPNG as well as non-PPNG strains, were 100% resistant to
Septrin. With Penicillin, only 72 non-PPNG strains were inhibited at a
MIC of 32.0 mg/L. Erythromycin, Tetracycline and Spectinomycin inhibited
all 300 selected PPNG strains at MICs of < or = 32.0 mg/L., < or = 4.0
mg/L. and < or = 16.0 mg/L., respectively. DISCUSSIONS AND CONCLUSIONS:
There is a high incidence of PPNG in Ghana. A review of the current
Gonorrhoea Treatment Regimen needs be undertaken. Suggestions to this
end have been made.
